All care management options include a Medical Surgical History Review during the first visit. The nurse will review the patients’ past medical and surgical history to identify potential risks, allowing for a tailored care plan. This allows the nurse to provide informed care, and allows physicians to make informed decisions when changes in a patient’s condition is found. Below, you will find detailed information about what is included with each care management option.

Evergreen is based out of Norridgewock, ME. Although I serve the entire state, a $25 travel fee applies to visits more than 45 miles from Norridgewock.

Wellness Checks

$50.00/visit

This is a good option for you if:

You would like as-needed nursing support without a monthly commitment, your healthcare needs fluctuate, and you are experiencing an exacerbation of an illness

This option includes:

  • Vital signs check

  • Assessing heart, lung, and bowel sounds

  • Head to toe skin check

  • Status update to family

  • One-hour visit

Respite Care Visits

This service is beneficial for caregivers who need time to tend to needs in their personal lives. This service is provided on an hourly basis.

$25/hour

Accompaniment to Healthcare Appointments

This service provides support to patients who may need assistance during medical appointments. The nurse assures patient needs are communicated to the healthcare provider, can explain medical jargon and complex plans to the patient, provide emotional support, assure follow up visits are scheduled, and provide an update to the family if requested. This service is provided on a per appointment basis.

Chronic Care Management

$125.00/month

This is a good option for you if:

You are managing multiple health diagnoses, are at risk for hospitalization due to health decline, have questions about your medication regimen, wound care, or medical devices, would like to give your family peace of mind by having scheduled nurse visits

This option includes:

  • Vital signs check

  • Assessing heart, lung and bowel sounds

  • Head to toe skin check

  • Reconciling medication lists

  • Assuring necessary physician appointments are scheduled

  • Communication with doctor’s offices as needed

  • Ongoing education regarding diagnoses/medications/care and use of medical equipment

  • Monthly updated care plan provided to patient and family. This includes a master medication list and comprehensive information from all disciplines (Primary care, cardiology, urology, nephrology, wound center, etc.…) into one central and easy to understand packet.

  • This includes two visits per month scheduled bi-weekly, and an as needed Wellness Check included if requested by the patient or family. These are one-hour visits. This also includes one accompanied visit by the nurse to a physician appointment if needed.

Transitional Care Management

$175.00

This is a good option for you if:

You are discharging home from a hospital or healthcare facility, and have a new diagnosis, new medical equipment, are post-surgery, have questions about your ongoing care, or simply want to make sure there are no gaps in your care.

This option includes:

  • Nurse meeting at the hospital, nursing home, or other facility for discharge day to review instructions with the discharging care team and family

  • Nurse will assist in helping the patient settle back into their home comfortably, and provide education on how to use and manage new medical devices such as: Urinary catheters, Colostomy or Ileostomy bags, Tracheostomies, Mobility devices, Wound vacs, Surgical wounds, Diabetic ulcers, Oxygen, Nebulizers, Diabetic supplies, Feeding tubes, etc.… as prescribed by their provider upon discharge

  • The nurse will also provide education on how to safely care for wounds such as surgical wounds or diabetic ulcers

  • Vital signs check

  • Head to toe physical assessment including a thorough skin check if necessary

  • Reconciling medication lists

  • Assuring follow up physician appointments are scheduled

  • Communication with doctor’s offices as needed

  • This option includes two visits to the home: one on discharge day, and one during the first week home – up to 3 hours each visit

Transitional Care Management PLUS

$275.00

This is a good option for you if:  

You are discharging home from a hospital or healthcare facility, and have a new diagnosis, new medical equipment, are post-surgery, have questions about your ongoing care, and may not have family close by to attend medical appointments with you or are not able check in with you in person regularly

 Includes everything within the Chronic Care Management package, as well as:

  • One additional scheduled visit for a total of (3) visits – Two during the week of discharge (including day of discharge), and one the week following discharge – Up to 3 hours each visit

  •  One Wellness Check as needed by request within the month following discharge

  • Accompaniment by nurse to one physician follow-up appointment within the month following discharge
